  1. Click the Edit tab above
  2. Copy everything below these directions
  3. Click Cancel down below
  4. Go to your new page
  5. Click the Edit tab
  6. Paste in this format
  7. Replace the ALLCAPS items (except TOC) with your text.
  8. Please do NOT, under any circumstances, change or move the Author/Illustrator Form page itself!
